Below are links to download commonly used forms at St.
John. These forms may be printed, completed, and turned into the church
office for handling by the church office.

- Mailings Word 6/95 format

Acrobat format
- If you have a mailing that needs to be sent to several
people in our congregation, fill out this form and attach it to a
(preferably) typed copy of your letter or flyer. Be sure to include a
list of all people this mailing needs to be sent to.
- Work Order Word 6/95 format

Acrobat format
- If you need to enlist the office staff's assistance in fulfilling a
work order, use this form
- Catalog Order Word 6/95 format
|
Acrobat format
- Specify the items you need ordered, the number of
items needed, what catalog they can be ordered from and the catalog's phone
number.
- Facilities and Equipment Request Form

- Requests for room reservations, calendar date, audio-visual
equipment, tables and chairs

and Medical Release Form

- This form is filled out for all of our basketball players,
participants in Vacation Bible School and Youth events.
- Reimbursement Forms
| Acrobat format
- If you need reimbursement for anything purchased for the church,
please fill out this form and put it in Genie's mailbox with your
receipt and the purpose of the purchase.
- Property
Use Agreement
| Acrobat format
- Any outside group using our facilities needs to fill out this form
and return it to the office prior to using the facilities.
Instructions on Downloading
1. Click the link below--IE will open in Word; Netscape will
offer to save or open in Microsoft Word
or click the Acrobat format link to open in
Acrobat reader.
2. If the browser cannot open the file, right-click on
the appropriate link below and choose save (Save Target As or
Save Link As)
3. Open the directory where form was saved and
double-click the filename to open
4. Follow instructions on the form and complete all
parts
5. Turn completed form with any attachments into the
church office
